HEALTH INSURANCE BENEFIT PLANS

Medical Coverage is provided for all regular full-time employees (30+ hrs./wk) through Health Net of Oregon. The co-pay for an office visit is only $25.00 when using a physician from the Preferred Provider list. Hospitalization is covered at 80% when in a participating facility.
Thousands of providers participate in the Health Net PPO system. Employees can find conveniently located physicians and facilities in all specialties.
This plan also includes coverage for all of the following:
Vision Coverage Co-pay of $10.00 for exam and small co pays for lenses & all the extras such as tints and UV coating.
Prescription Coverage Co pay of $10.00 for Tier 1 drugs (generally generic drugs), $35.00 for Tier 2 drugs and $50.00 for Tier 3 drugs (generally name brand drugs).
Mail Service Prescription Drug Program A three month supply of your medications delivered to your door for only two co-pays. A 1/3 savings!
Mental Health and Drug & Alcohol Rehabilitation Services Covered at parity with other health issues.
Alternative Care Coverage Chiropractic, naturopathic and acupuncture visits have only a small $15.00 co pay. Massage therapy has only a $25.00 co pay.

401k Pension Plans - A 401(k) Pension Plan is provided for all employees of participating groups who have been on the payroll at least three months.

Paid Leave, Holidays & Vacation Time - Minimum levels of paid time off are set by Magus and clients may define additional time appropriate to their needs and industry.
